Contract Length:Temp to Perm

Shift Details: Clinic begins at 7am and closed at 1930. Also open on Saturdays 9-2. Work hours can be flexible within the requirements of the FTE. We are in need of provider help and access every day except Thursday

Amount of Coverage Needed per Month:
Patients per day: 18 PPD
Patient Population/Age: 6 years +, Majority of patients over 45
Department Description: We are recognized as a Patient-Centered-Medical-Home for nearly the last 20 years. Family Primary Care practice accepting patients age 6 thru end of life. However, pediatric population is only 40 patients between 6-17 total census is just over 5600.
