IMPaCT Community Health Provider Partnership

We’re looking for warm-hearted primary care docs (MD/DOs) to provide general supervision for Community Health Workers who work in your local community.

If you work with us you will:

Dedicate blocks of time on your schedule to do real-time telehealth visits with patients as they are outreached by Community Health Workers in the community. Imagine being in your pajamas on a Tuesday afternoon doing a telehealth visit with a recent widow at a senior center, or a new immigrant at her church. You’ll get a quick hand-off from the Community Health Worker who will tell you about who your new patient is as a person. Then, you’ll be able to evaluate the patient’s needs and determine whether they would be eligible for Community Health Integration services: do they have a medical condition that’s being affected by a social need?
IMPaCT will integrate our Community Health Worker documentation app with your practice’s EHR instance. On your own time, you’ll review notes that Community Health Workers push into your EHR that describe the social support, advocacy and navigation they provide to patients. You’ll read about patients who reconnected with estranged families, found a job after incarceration or got clean from an addiction. You’ll drop brief monthly notes and bills for each patient tallying up the services that Community Health Workers provided.
If patients don’t already have a trusting relationship with a primary care provider, you’re welcome to be their doc. But you don’t have to. Community Health Workers will help each patient find a primary care home that works well for them. You will be asked to respond to messages from Community Health Workers regarding urgent clinical issues that may arise for a new patient before they’ve been able to establish primary care elsewhere.
You will work with a group of Community Health Workers who will send you new patients. You will be able to earn $100 per hour with additional revenue to cover practice costs. You would also receive revenue for any E/M visits you do to address urgent clinical issues as new patients bridge to primary care. Not counting the time required for these additional E/M visits, we estimate that you can do the work required in 5-10 hours per week.
Your practice will be responsible for the billing and collections. IMPaCT Care will take care of paying the local organizations employing Community Health Workers. Community Health Workers and our practice liaison will need limited access
