Family Practice
Fulfil has partnered with a federal nursing facility in central Kentucky with a contract assignment for a Family Medicine Nurse Practitioner or Physician Assistant to assist their team.
Location: Wilmore, KY
Schedule: 8-hour shifts
-Call Requirement: 1-2 weeks per month
-Will conduct routine visits and follow-up visits
-Duties: Typical family medicine/primary care responsibilities
-Paid travel & expenses: Full Travel
-Kentucky License needed
-DEA Required
-AHA ACLS; AHA BLS Required
Qualifications and Requirements:
-The APP must be qualified to provide services in the Commonwealth of Kentucky and hold a valid license to practice in the state.
-The APP must not be an excluded Medicare/Medicaid provider.
-A criminal background check, abuse registry check, and drug testing are required.
-The APP must comply with all facility policies and procedures.
-Services must be provided to all residents without regard to race, color, creed, national origin, age, sex, religion, or disability.
-The Contractor shall not bill facility residents unless specifically authorized to do so as outlined in the document.
APP Services must adhere to all relevant state and federal guidelines and regulations that pertain to skilled nursing facilities. The APP will also comply with regulations set forth by the Centers for Medicare and Medicaid Services (CMS), and provide care within their scope of practice, which includes but is not limited to the following duties:
-Conduct nursing facility rounds as needed, in accordance with applicable regulations.
-Utilize PointClickCare software for patient management and documentation
-Participate in the existing call service rotation (approximately 1-2 weeks of on-call service per month)
-Coordinate emergency care for facility residents who may be admitted to a hospital.
-Facilitate the transport of facility residents to the nearest acute care facility when necessary.
-Oversee and manage the care of facility residents in a skilled nursing facility setting, including supervising their medical care.
-Participate in the assessment and care planning for residents, monitor changes in their medical status, and provide consultation or treatment when contacted by the facility.
-This scope also includes, but is not limited to, the following activities:
--Prescribing medications and therapy.
--Ordering transfers to the hospital when necessary.
--Conducting required routine visits and completing follow-up visits.
--Ordering, interpreting, and evaluating diagnostic tests to assess patients’ clinical problems and healthcare needs.
--Documenting behavioral and/or physical findings and formulating plans and prognoses in the PointClickCare electronic health record system.
--Collaborating with physicians and other healthcare professionals to prepare comprehensive patient care plans.
--Submitting healthcare plans and goals for periodic review and evaluation by a physician.
--Prescribing or recommending non-scheduled medications (e.g., physical therapy, inhalation therapy, or other related treatments).
--Referring patients to physicians or specialized health resource
