Job description

- Any state license accepted
- Coverage dates: ASAP – 6-month initial contract, with the ability to extend
o Tentative credentialing timeframe: ~10 weeks
o Mon-Fri: 40hours
- About 15 patients/day
- Adult patients only
- Primary Care cases, sick/well visits, some chronic care coverage like hypertension/diabetes, light trauma cases such as suturing
- ANY State license

• Any state license is accepted
• 6 month initial contract, with the ability to extend
• 15 patients ppd (adult patients only)
