Any supervision? All CRNA practice
Types of cases required: General, Ortho, ENDO, Podiatry- (every other week)
Spinals, blocks, epidurals - must be proficient: Yes
Case logs required: Yes, proof of epidurals, spinals and blocks
Number of providers during the day: 3
Number of ORs: 2
EMR: EPIC
Credentialing Timeframe: 120 days
Licensure Requirements: ACLS, BLS, PALS

• 13 weeks with option to extend
• On call required
