• Length of commitment: 3-6 months with the option to extend or transition to Perm

o June 2, 2025 (8:30a-5p) clinic coverage
o June 2, 2025 (night call, 4p-8a)
o June 3, 2025 (8:30a-5p) clinic coverage
o June 6, 2025 (8:30a-5p) clinic coverage
o June 6, 2025 (night call, 4p-8a)
o June 7, 2025 – June 9, 2025 weekend call (starts at 4p on Friday June 6th to 8a on Monday June 9th)

o June 16, 2025 (night call, 4p-8a)

• Schedule Details: 830a-4p for week day coverage
• Support staff: 1 app 1 RN 1 MA
• Patients per day: 15-18
• Patient population/age: 18yr and older
• Department description: Hematology/Oncology Clinic
• Outpatient Clinic/Call Responsibilities: Checking EPIC In Basket Daily/Empty daily, Placing own orders (nurses will help if needed), Sign outpatient notes by EOD or End of following day. Report off to next provider in On Call Rotation, Take after call coverage when scheduled, Round on any assigned inpatients in the morning-repeat as needed and complete consult notes by EOD.

• Required: Board Certified, Active OH license or IMLC, DEA, 1 Year experience
