Job description
Job Details:
- Conduct 5–6 home health assessments per day
- Part-time and full-time options
- Paid for a full day, regardless of patient volume
- Perform annual risk assessments and review of chronic conditions
- No treatment or prescribing – assessment-only role
- Escalate urgent findings to the patient’s PCP
- If applicable, conduct A1C tests only (other screenings left for the patient)
- Travel required; mileage reimbursed daily
Two shifts potentials: 8am-5p and 9a-6p.
Duration: 90-day assignment with option to extend
Qualifications:
- Must be Board-Certified FNP or ACNP
- Must be licensed in NJ
- Mileage will be reimbursed daily
- Will not cover travel outside of the daily mileage reimbursement, so local candidates are the best option for this job
Home visits across Ocean County
- Start Date: as soon as privs are issued
- Assessment Role in Orange County
- Part-time 3–4 shifts per week; Full-time 40 hours per week
