Bergen, Hudson, and Essex Counties - Part time Home Health Locums Assignment in New Jersey

Job description

- Location: Home visits across Bergen, Hudson, and Essex Counties.
- Scheduling: Must be available for 8 hours shifts. One-hour lunch break included
- Schedules are released 2–3 weeks in advance
- Monday through Saturday schedule available (flexible within that window)
-Part time available - 3-4 shifts per week


Job Details:
- Conduct 5–6 home health assessments per day
- Paid for a full day, regardless of patient volume
- Perform annual risk assessments and review of chronic conditions
- No treatment or prescribing – assessment-only role
- Escalate urgent findings to the patient’s PCP
- If applicable, conduct A1C tests only (other screenings left for the patient)
- Mileage reimbursed daily
- Equipment: iPad, Blood pressure cuff, Lab kits for patient use
- Credentialing: 1–2 week turnaround
- Must have an active NJ license and NJ Medicare number

Qualifications:
- Must be licensed in NJ
- Must have NJ Medicare number active
- Board-Certified Physician Assistant or Nurse Practitioner
