On Call, Call Back Requirements: none
Weekend Rotation: none
Holiday Requirements: none
Floating Requirements: May rarely float to Metro A
Patient Service Area (PSA) : Continuing Health PSA

JOB DESCRIPTION
To be minimally qualified for this position, your candidate MUST HAVE the following:
- 1 year SLP experience (any setting)
- IL SLP license and BLS in hand at time of submission
- Experience with: Swallow Studies, time management, effective communication, patient/family education, cognitive training, organizational skills

Home Health, covers majority of Kendall, Kane and Will counties with some DuPage county.
Adult patients only (rarely may see some adolescents) presenting with stroke, cognitive issues
Will see approximately 5-6 patients daily
EPIC EMR - experience is helpful
Professional attire.
Candidate must have own reliable transportation.
